Goody bag stuffers request form 

1. Allow at least 60 days for processing and shipping via UPS Ground.
2. Be sure you have a street address for delivery within the contiguous 48 States (no P.O. Boxes).
3. Download and print the Overview & Application Form.
4. Be sure you complete all information and select the items in order of preference (if requesting multiple items).
5. Make a copy for your records.
6. Submit to Mothers the original completed and signed Application Form and one copy of your Event Flyer and other supporting materials, if applicable.
7. If you completed all steps as asked, simply wait until your shipment arrives.
