For clubs and other organizations in need of goody bag inserts for an upcoming show or event that is 90 days or more in the future, we are glad to provide the following:
If you’d like any of these materials in reasonable quantities, we are glad to help. We do not ship samples of products, but we may be able to provide limited amounts of product under special circumstances. Please download, complete, and submit the simple form below along with your event flyer and select your order of preference (incomplete submissions will be not be processed).
Please follow the steps below:
Step 1: Allow at least 60 days for processing and shipping via UPS Ground.
Step 2: Be sure you have a street address for delivery within the contiguous 48 States (no P.O. Boxes).
Step 3: Download and print the Overview & Application Form.
Step 4: Be sure you complete all information and select the items in order of preference (if requesting multiple items).
Step 5: Make a copy for your records.
Step 6: Submit to Mothers the original completed and signed Application Form and one copy of your Event Flyer and other supporting materials, if applicable.
Step 7: If you completed all steps as asked, simply wait until your shipment arrives.
